How to Correctly Fill Out a Illinois Vehicle Title
1. Locate the Title
Ensure you have the original title document in good condition, without any alterations or damages.
2. Complete the Seller’s Section
In the seller’s section, provide the following information:
- Seller’s full name and address.
- Sale date and sale price.
- Odometer reading at the time of sale.
- Seller’s signature.
3. Complete the Buyer’s Section
In the buyer’s section, provide the following information:
- Buyer’s full name and address.
- Buyer’s signature.
4. Use an Odometer Disclosure Statement
If the vehicle is less than 10 years old, an Odometer Disclosure Statement is required.

5. Double-Check for Accuracy
Ensure all information is accurate and complete. Any errors or omissions can delay the title transfer process.
6. Notarize the Title (if required)
Some transactions may require notarization. Check if your transaction needs a notary and, if so, get the title notarized.
7. Submit the Title
Submit the completed title along with any required forms and fees to your local Illinois Secretary of State facility.
